IMPORTANT DATES AND REMINDERS

  1.   ALL PLAYERS MUST HAVE HAD A PHYSICAL EXAM SINCE OCTOBER 17, 2014.

IT IS THE PLAYERS RESPONSIBILITY TO HAVE THE COMPLETE PHYSICAL EXAM TO THE SCHOOL NURSE BY THE FIRST DAY OF PRACTICE.

  •  All parents should also have a copy of their athlete’s physical on file at home.
  • IF THE NURSE DOESN’T HAVE AN EXAM ON FILE THE PLAYER CAN’T PRACTICE UNTIL ONE HAS BEEN TURNED INTO THE NURSE.  THE PLAYER STILL MUST REPORT TO PRACTICE.

               Every player must participate in at least 10 practices before they can play in their first game.

  1.  PRACTICE FIELD:  The football field is located directly behind the Middle School.   Students will be ready to be picked up after practice (and an optional SHOWER has been taken) by 6:00 P.M. (5:15 P.M. on Fridays)
  1. FEES/COSTS: $139.00 Participation Fee.  This fee needs to be paid before the first day of practice. $100.00 Summer Camp (optional) Read more about it later on in the packet.
  1.  PRACTICE EQUIPMENT:  The school will supply shoulder pads, knee pads, helmet, practice pants, mouth guard, game pants and a game jersey.  

              ALL PLAYERS MUST PROVIDE: THEIR OWN SHOES, SOCKS, T-SHIRT, 5 PIECE HIP/THIGH PAD GIRDLE AND PRACTICE JERSEY.  

              (All boys attending summer camp will receive a T-SHIRT, PRACTICE JERSEY AND SHORTS.)

  1.  FOOTBALL IS A TEAM SPORT.  PLAYERS WORK TOGETHER TOWARDS A COMMON GOAL.  WE NEED THE BEST ATHLETES IN OUR SCHOOL ON OUR TEAM.  THROUGH HARD WORK AND COOPERATION, ATHLETES WILL LEARN TO CONTROL THEIR BODIES AND THEIR MINDS TO BE THE MOST COMPETITIVE TEAM POSSIBLE.

WE ALSO BELIEVE IT IS IMPORTANT TO MAKE CLEAR THAT SINCE THIS IS THE FIRST

TIME MANY OF THE BOYS HAVE PLAYED SUCH A COMPLICATED TEAM SPORT, WE AS

COACHES, WILL STRIVE TO ENCOURAGE THEM, TEACH THEM THE IMPORTANCE OF

WORKING TOGETHER AND HELP THEM FIND REAL IMPROVEMENT IN THEIR ABILITIES

AS THE SEASON CONTINUES.  WE WILL TRY TO PLAY ALL OF THE BOYS AS OFTEN AS

WE POSSIBLY CAN.  ATTENDING ALL PRACTICES, HARD WORK, DETERMINATION,

COOPERATION, AND A GOOD ATTITUDE ARE QUALITIES THE COACHES WILL BE

LOOKING FOR FROM THE BOYS. YOUR POSITIVE ENCOURAGEMENT AND SUPPORT AS

A PARENT ARE ALSO CRITICAL TO THE SUCCESS OF THE PROGRAM.

What are camp, back-to-school and sports physicals?

While specific requirements vary by school, camp or sports association, routine camp, school and sports physicals are an opportunity for healthcare providers to discuss important health issues with children and their guardians. Physicals can help ensure that the individual is healthy and physically prepared to join in activities. During the exam, the healthcare provider will review the individual’s complete health history and ensure that he or she is up-to-date with current immunization recommendations for their age.
At Healthcare Clinic, we also include information about preventing and recognizing the signs of concussions in youth athletes.

Who needs camp, back-to-school and sports physicals?

Contact your school, camp or sports association for specific recommendations in your area. Many states require these examinations at least once a year or before participating in any physical activity, camp, or sport.

Before you come in, contact your Healthcare Clinic to make sure they are able to complete any forms/letters required by your school/camp/sports association. Be sure to bring any required forms with you.

Camp, sports and school physicals are available at Healthcare Clinic for patients ages 21 and under. Minors must be accompanied by an adult chaperone or guardian and have parental/guardian permission.

Most camp, back-to-school and sports physicals are not covered by insurance. Payment via cash, debit or credit card is due at the time of service.
