Dear Parents/Guardians,
Thank you for completing the Welcome to RMS Event participation form to let us know if your child will be attending on Monday, August 9th from 9:00-11:00. So far, it looks as if almost 70% of our 6th grade class will be with us that morning. If you have not completed the form, but your child still wants to participate, please complete the participation form.
Once again, for those who are eligible for school bus transportation, our buses will run all routes that morning to and from the event. Once the PowerSchool portal opens at the beginning of August, students will be able to view their schedules as well as get information about their bus route (number, pickup and drop-off locations and times). For planning purposes, you can plan that the pickup time listed in PowerSchool will be exactly one hour later on August 9th. The drop-off time will be approximately four hours earlier than the time listed in PowerSchool for a regular school day.
For example, if the pickup time listed in PowerSchool is 7:38 a.m. and the drop-off time is 3:24 p.m, you should plan that the August 9th pickup time will be 8:38 a.m. and the drop off time will be approximately 11:24 a.m. Since only 6th grade students will be riding the bus on this day, the routes may go a bit quicker, so please plan accordingly and make sure students are at the bus stop 10 minutes early on this day.
Other students who would typically walk, ride their bike or get dropped off will do that as they would on a regular school day. The RMS doors will open at 8:45 that morning for students who are being dropped off or who are walking or riding their bikes. Students should plan on having access to their schedule (either a printed copy or one accessible on their phone) on that morning. We will have schedules available for students who do not bring theirs. Students will not need any other materials or supplies on that day.
Students will get to experience arrival and dismissal procedures and get a tour of the building, with an emphasis on the classrooms they will need to get to and from during the first semester of school. In addition, students will get to see how PE and lunch work at RMS as well as get their questions about middle school answered.
Our annual ‘registration and locker move-in day’ will be held the following day (August 10th) for all students from 9:00 a.m. until 6:00 p.m. On that day, students will get their fall school picture taken, they will be able to walk their schedule, pay any unpaid student fees, and get questions answered from staff members.
We hope that this experience will allow our newest RMS students to feel more comfortable as we head into the new school year! We hope you have a great summer, and we can’t wait to meet our new 6th graders in August!

A Message from the RMS Health Office To Incoming 6th Grade Families
Dear Parents of current 5th graders ~
WELCOME TO Rotolo Middle School! Your student’s first day of middle school will be here before you know it!
For your student to begin middle school in August 2021, there are a few health requirements your student will need.
All 6th grade students are required to have a 6th grade physical and an immunization record complete with a Tdap and Meningitis vaccine on the State of Illinois Health Form. This physical must be dated AFTER August 18th, 2020. This physical must be received in the Rotolo Middle School Nurse’s Office before the first day of school. Your student will NOT be allowed to start school without this physical and two immunizations (no exceptions). All 6th grade students are required to have a dental exam done. This dental exam is due May 15, 2022. It can be completed from November 15, 2020 – May 2022. Dental Exam Form (English)

Any student requiring medication during the school day is required to submit a “Medication Administration Form”. No medications other than inhalers* and Epipens are allowed to be self carried by the student. All medications (prescribed and over the counter) including self carried inhalers and Epipens require the completed Medication Administration Form to be on file in the Nurse’s Office.
*For inhalers only, a copy of the prescription label may be submitted instead of a medical provider signature.
These forms can be submitted upon completion to Rotolo Middle School. They can be faxed to 630-937-8701, scanned and emailed to us, dropped off at Rotolo Middle School, or sent to the RJAC (Administration Office).
